Teacher Training Program Application
Please print out and complete this form. Return it with the $108 application fee it to Karina Mirsky no later than 30 days before the start of the training. This application fee pays for the printing of the manual, and will be credited toward the total tuition payment.
By submitting this application you are agreeing to the terms and conditions of this Teacher Training Program's Contract/Catalog of Policies.
All tuition and fees paid by the applicant shall be refunded if (by the unlikely chance) the applicant is not accepted to the program. All tuition and fees paid by the applicant shall be refunded if requested within three business days after the approval of this application. An application fee of not more than $25 may be retained by the school if the applicant withdraws from the program with 3 business days of application approval. See the Catalog of Policies for information about refunds after participation in the program begins. All refunds shall be returned within 30 days.
